Welcome to the 2010 Camper Forms & Waivers page, and thank you for enrolling in ADTC 2010!

Our goal is to make this page your best resource for everything you need to prepare for camp. We are in the process of updating and uploading all of the forms we require and information you desire. As forms are added, we'll email all registered families. In the meantime, please feel free to get started. And if you need something right away, let us know and we'll do our very best to get it to you ASAP!

A handy guide to help you through the required camper forms and waivers submission process

The following forms are REQUIRED of all ADTC 2010 campers. Forms are due by June 15, except forms 3 & 4 which can be turned in anytime before - or at - check-in. (PS: we'll have printed copies of both forms 3 & 4 at check-in, so you don't have to bring your own unless you want to.)

Note: Copies of immunization records from health-care providers, schools or state/local governments are acceptable.

Note: Copies of standard physical forms from health-care providers, schools or state/local governments are acceptable.

All of our online forms are encrypted and secure. If you experience problems with any of our online forms, please contact the office: 1-866-383-ADTC or email ADTC@ASDance.com.

How to submit your printed forms:

Please submit Forms 2, 7 & 8 plus your camper's health insurance card all together whenever possible.

Submit by June 15: Form 2, Form 7, Form 8, and a front/back copy of the camper's health insurance card.

Submit before (or at) check-in: Form 3 & all subforms

Next-Best Choice:

Scan & Email: Scan in all documents (pdfs or jpgs), then email them to ADTC@ASDance.com. Put “HEALTH FORM SUBMISSION” as the subject of your email, and your camper’s full name, camp location and camp dates in the body of your email. (Don’t forget to attach your forms!)

Third Choice:

Fax: Fax in all documents to 866-383-2382 OR 415-666-2312. (They come to the same place.)

Please fax in your forms ONLY if neither uploading nor emailing is possible.

Put your camper’s full name, camp location and camp dates on your cover page.

Last Resort:

If you cannot do any of the above, hold onto your forms and bring them with you to check-in. Call the office at 866-383-2382 OR 415-666-2312, and follow the prompts to connect to your camp location, to tell the camp director of your plans.
