REGISTERED CAMPERS' HANDBOOK (.pdf)

All campers and parents should read each applicable section of our 2011 Registered Campers' Handbook before coming to camp (click link above). Here's what is inside:

1. ADTC = More Than Dance! This section is all about where we're coming from, why we do it and what we want YOU to get out of it! (Includes: ADTC Goals for Campers, ADTC Character Development Program, Diversity/DiverCity Initiative, Camping Program Philosophy, ADTC Camp Code of Conduct)

2. ADTC Camp Policies. This is all the stuff you've got to read, know and agree to before you come to camp. There won't be a quiz, but we're going to hold you to it. (Includes: General Policies, Health Policies & Procedures, Policies Regarding the Internet & Other Technologies, Policies Regarding Keeping In Touch Outside of Camp)

MANDATORY CAMPER FORMS

The following forms are REQUIRED of all ADTC 2011 campers. Forms are due by June 15, except forms 3 & 4 which can be turned in anytime before - or at - check-in. (PS: we'll have printed copies of both forms 3 & 4 at check-in, so you don't have to bring your own unless you want to.)

How to submit your printed forms:

Please submit Forms 2, 7 & 8 plus your camper's health insurance card all together whenever possible.

Submit by June 15: Form 2, Form 7, Form 8, and a front/back copy of the camper's health insurance card.

Submit before (or at) check-in: Form 3 & all subforms

#1 Best Choice:

Upload your completed & signed pdf (or jpg) forms to the ADTC Upload Center!

Next-Best Choice:

Scan & Email: Scan in all documents (pdfs or jpgs), then email them to Info@DanceADTC.com. Put “HEALTH FORM SUBMISSION” as the subject of your email, and your camper’s full name, camp location and camp dates in the body of your email. (Don’t forget to attach your forms!)

Third Choice:

Fax: Fax in all documents to 866-383-2382 OR 415-666-2312. (They come to the same place.)

Please fax in your forms ONLY if neither uploading nor emailing is possible. Put your camper’s full name, camp location and camp dates on your cover page.
