Please complete this contract and then call the ADTC office at 866-383-ADTC (2382) to inquire about availability for your preferred date/s. Next, complete the "Estimated Party Cost" form (located in the Birthday Party Info & Planning Guide). Remember that we must receive your contract & 50% deposit at least 2 weeks before your party date!
