All campers registered to attend American Dance Training Camp must submit either ADTC's standard health form OR their school/sports medical form (as long as ALL required information is coverd). Health forms are due by June 1, 2008. Please note: camper must have had a physical within two years of the dates she will attend camp! Also note: Alumni campers must submit updated health forms each year - ADTC does not keep camper health records on file!
